In patients with a history of radiation therapy for extracranial or extraaxial tumors, radiation necrosis in the brain may be identified using magnetic resonance (MR) imaging supported by perfusion MR imaging, MR spectroscopy, and positron emission tomography (PET), as outlined in subsequent sections of this article. 2016-01-07 · Purpose This meta-analysis examined roles of several metabolites in differentiating recurrent tumor from necrosis in patients with brain tumors using MR perfusion and spectroscopy. Methods Medline, Cochrane, EMBASE, and Google Scholar were searched for studies using perfusion MRI and/or MR spectroscopy published up to March 4, 2015 which differentiated between recurrent tumor vs. necrosis in Perfusion imaging can play an important role in determining the malignancy grade of a CNS tumor. Perfusion depends on the vascularity of a tumor and is not dependent on the breakdown of the blood-brain barrier.
Perfusion MR Imaging in Tumor Characterization . Different tumor types and grades differ in their perfusion characteristics. The higher vascularity of brain neoplasms is most commonly quantified with perfusion MR techniques in terms of the cerebral blood volume of the tumor. 2.1 Gliomas

MR perfusion with ASL utilizes the magnetic labeling of arterial blood water through the use of an RF pulse. After this proton labeling, a period of time referred to as the postlabeling delay is necessary to give the labeled blood time to reach the brain parenchyma. Vessel-specific wavelet magnetic resonance perfusion (wavelet-MRP) maps were calculated using the wavelet transform (Paul wavelet, order 1) of each voxel time course. Five different aspects of image quality and tumor delineation were each qualitatively rated on a 5-point Likert scale.

Using the gradient EPI technique, perfusion-weighted MR images (PWIs) were obtained in four patients from the data of dynamic MR images. The maximum relative cerebral blood volume (rCBV) ratio was calculated between the tumor and contralateral white matter in two cases. Perfusion results were also significantly different in patients with stroke (ratio cortex/white matter affected hemisphere: MTT 1.058 vs 0.784; p < 0.001), dementia (ratio cortex/white matter left hemisphere: rBVc 1.152 vs 1.795, p < 0.001; right hemisphere: rBVc 1.396 vs 1.662, p < 0.05) and brain tumors (ratio cortex/whole tumor rBVc: 0.778 vs 0.919, p < 0.001 and ratio cortex/tumor hotspot

MRI – Blood Brain Barrier Imaging: Blood-Brain-Barrier Imaging in Brain Tumors: Concepts and Methods Neurographics 2012 OBJECTIVE: Arterial spin labeling (ASL) magnetic resonance (MR) perfusion is a noninvasive and repeatable method for quantitatively measuring cerebral blood flow (CBF). This study aims to compare measurements of ASL-derived CBF with dynamic susceptibility contrast (DSC) MRI in the assessment of enhancing brain tumors (primary and metastatic), with an aim to use ASL as an alternative to DSC. Recent studies have indicated that perfusion MR imaging, both evaluation of relative CBV by using a DSC technique and CBF by using a PASL technique, can help predict the grade of astrocytoma and the likelihood of rapid progression. 9 –30 Some studies have also suggested that perfusion imaging may be helpful in differentiating treatment-related effects versus tumor progression.
